HAP partners with best-in-class vendors to make sure our patients get the care they need. Consult your statement of benefits to see which programs are available to you.

HAP Behavioral Health: Protocol is available to our members who need behavioral health support or crisis response after hours.

Carelon Health Palliative Care: Carelon Health specializes in providing an extra layer of support to patients facing a serious illness. This partner provides home-based palliative care to our members with chronic illness who are near the end of life.

HAP Clinical Care: HAP and partner Matrix Medical provide expert care and health services to plan members. A network of over 5,000 clinicians assist patients with health and safety assessments and identifying and closing care gaps. Both telehealth and in-home services are available. 

HAP Companion Care: HAP connects plan members and their families with real people for help with companionship, everyday tasks, transportation, and more. Assistance is available both in-person and over the phone.  

HAP Dental: When you select a HAP health plan, you can add high-quality dental coverage through our partnership with Delta Dental. Both pediatric-only and adult coverage are offered. 

HAP Diabetes Care: HAP's Diabetes Care Connection (DCC) Program, powered by Henry Ford Health, works with those with diabetes or at risk for developing diabetes. A variety of educational services, programs and support groups are offered for patients of all ages. HAP Medicare Members with pre-diabetes also have access to the National Kidney Foundation of Michigan's Diabetes Prevention Program, which helps people at risk learn how to make lifestyle changes and stick to them. 

HAP Digital Wellness Manager: HAP provides members with free health and wellness tools and personalized interactive programs to help achieve individual health goals. Download the free Wellness at Your Side app on Google Play or the App Store. 

HAP Fitness: HAP partners with SilverSneakers® to offer a best-in-class fitness benefit with access to thousands of participating locations nationwide. On-demand fitness classes and live virtual classes are also available. Members are able to enroll in multiple gym locations at the same time. 

HAP Flex Card: HAP and our partner NationsBenefits offer a comprehensive flex benefit card that provides members with a convenient way to pay for eligible items and approved services. Members receive a personalized debit card that gives them quick and easy access to funds loaded on a monthly, quarterly, or annual basis. 

HAP Hearing: HAP and our partner NationsHearing simplify the process for receiving hearing care by scheduling a no-cost hearing test, and if necessary, helps you select the most comfortable and effective hearing aids to meet your needs and lifestyle.  

HAP In-Home Testing: HAP offers access to credible, validated laboratory tests for chronic diseases that are initiated and collected by you in the comfort and convenience of your own home. Our partner Everlywell helps members complete testing and screenings and delivers the results. Our vendor Quest HealthConnect provides in-home bone mineral density exams and other in-home screenings.   

HAP Meal Benefit: Eligible HAP members have a $0 copay for 28 fresh, nutritious, ready-to-heat meals upon discharge after a hospital admission.

HAP OTC: This benefit lets you purchase over-the-counter items you may need using an allowance. There are three ways members can place their order: online, by phone or by mail. No matter which way you order, your items will be shipped directly to your door. 

HAP Palliative Care: HAP offers home-based palliative care to our members with chronic illness who are near the end of life. 

HAP PERS: HAP's personal emergency response systems (PERS) benefit includes in-home Medical Alert base units and On-The-Go systems with help buttons and monitoring to fit members' lifestyles and budgets. Our solution offers safety for members when they are at home or on-the-go with GPS location capability.  

HAP Post-Acute Care: Our partnership with naviHealth allows us to effectively and efficiently manage care transitions for members who are admitted to SNFs, LTACs, and in-patient rehab.

HAP Telehealth: See a HAP or Henry Ford Health doctor online 24/7 using your phone, computer or tablet.

HAP Travel Assistance: 24/7 travel assistance for HAP members who are travelling 100 miles or more from home for up to 90 consecutive days per trip. Services like critical care monitoring, emergency medical evacuation, and hospital admission support are included. 

HAP Vision: HAP plans offer high-quality vision coverage through our partnership with EyeMed, which provides easy access to a expansive network of providers across the U.S. including Henry Ford OptimEyes™, SVS Vision Optical Centers and Rx Optical®. 

Health Alliance Plan (HAP) has HMO, HMO-POS, PPO plans with Medicare contracts. HAP Medicare Complete Duals (HMO D-SNP) is a Medicare health plan with a Medicare contract and a contract with the Michigan Medicaid Program. Enrollment depends on contract renewals.
