HAP Medicare Advantage dental coverage with Delta Dental
Medicare Advantage dental
As a member with a HAP Medicare Advantage HMO1, HMO-POS1 and PPO2 plans, the following preventive dental services are covered per year:
- two dental exams
- two cleanings
- two fluoride treatments
- one brush biopsy
- a set of bitewing x-rays
Coverage for comprehensive services such as root canals, fillings, simple extractions, crowns and crown repairs are also covered.
You have access to the Delta Dental’s Medicare Advantage PPO™ network. You may also be eligible for coverage when traveling outside of Michigan. Call the customer service number on the back of your HAP insurance card April 1 through Sept. 30: Monday - Friday, 8 a.m. to 6 p.m. Oct. 1 through March 31: seven days a week, 8 a.m. to 8 p.m. to learn more.
1 HAP Medicare Advantage HMO and HMO-POS Plans: Services received from a nonparticipating dentist are not covered. You will be responsible for the full cost of the services.
2 HAP Medicare Advantage PPO Plans: Services received from a nonparticipating dentist may result in higher out-of-pocket costs. You will be responsible for the difference between Delta Dental’s payment and the amount charged by the nonparticipating dentist.
Visiting the dentist? Remember to use your HAP ID Card and your 11-digit HAP ID number when you visit the dentist.

The following plan may purchase additional dental coverage: HAP Medicare Connect (HMO), HAP Senior Plus Henry Ford Tiered Access (HMO), HAP MSU-HC Medicare (HMO), HAP Senior Plus (HMO-POS), HAP MSU-HC Medicare Prime (PPO), HAP Senior Plus (PPO) and HAP Medicare Explore (PPO). Our dental/vision buy-up packages includes one vision exam and $175 allowance for vision hardware such as contact lenses, frames or glasses. If you didn’t purchase this buy-up package but would like to, you can add it within 30 days of your effective plan date by calling customer service.
If you purchased this buy-up package you will receive a separate ID card from Delta Dental and EyeMed in the coming weeks.
