Keep your health on track

It can be difficult to manage a chronic condition such as diabetes or asthma. That’s why many of our HAP Medicare plans include a free care management program.

Care management provides education and support to help you follow your treatment plan and keep your daily health goals on track.

How will the care management programs help me manage my chronic condition?

Chronic and complex conditions can need many health care providers, prescriptions, appointments and treatments. 

As part of the care management program, we’ll develop an approach personalized for you. We’ll help: 

  • Coordinate your care 
  • Educate you about available services and resources 
  • Advocate for your safety, quality of care and cost-effective outcomes 

This program helps you understand your conditions and learn to manage them. 

How do I join HAP’s care management programs?

For more information about HAP’s care management programs or to enroll, call (800) 288-2902 (TTY: 711) or email caremanagement@hap.org. Your doctor, family member or caregiver may also refer you.

HAP Senior Plus (HMO-POS), HAP Medicare MedicalAccess (HMO), HAP Senior Plus Henry Ford Tiered Access (HMO), HAP Medicare Connect (HMO), HAP Medicare Superior (HMO), Henry Ford Select (HMO), HAP Senior Plus (PPO), HAP Medicare Explore (PPO), HAP Medicare Prime (PPO), HAP Member Assist (PPO), HAP Medicare Diabetes and Heart (HMO C-SNP), and HAP CareSource™ MI Coordinated Health (HMO D-SNP) has a contract with Medicare. Enrollment depends on contract renewal.
